Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester. The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients. An exciting community opportunity has arisen to join the Fylde Enhanced Primary Care Team as a Community Matron. The successful candidates will be registered nurses or therapists with significant experience in medical care and long term condition management. They will have gained a post registration qualification, and be in possession of, or be in a position to undertake, the extended non-medical prescribing qualification. The ideal candidates will be self-motivated with the experience to manage change, strong organisational skills, with the ability to make complex clinical decisions autonomously and be able to develop robust outcome measures for the service. If you would like to improve patient experience and reduce hospital admissions, whilst supporting the team of community professions forwards with the integration agenda then this could be the job for you.
