Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester. The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients. It is an exciting time to join the new Extensive Care Service within the Community Division at Blackpool Teaching Hospitals NHS Foundation NHS Trust, which is looking to appoint a 2nd Consultant Physician to work with our Clinical Lead and 2 GP colleagues.

We wish to further support the Extensive Care Service which has now been in operation for 12 months as a new model of care under the NHSE Vanguard Programme and therefore this appointment is crucial to providing that support to our staff and patients.

The Extensive Care Service is a MDT which consists of Consultant Physicians, GP's, Advanced Practitioners, Pharmacists, Clinical Care Coordinator, Pharmacy Techs, Well Being Support Workers and Primary Care Assistants.

The service aims to support patients to develop their own health and well-being goals and then work with them to achieve these. The service works by helping to coordinate care provided to patients so that it is delivered in the community setting and provided by one core team. Ultimately we are working with patients to help reduce their A&E and NEL admissions along with a reduction in outpatient appointments.
