Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester. The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients. An exciting opportunity has arisen for two full-time health and wellbeing support workers in Wyre Enhanced Primary Care (EPC) and one full-time in Fylde Enhanced Primary Care.

We are looking for caring, motivated individuals to join our neighbourhood care teams to support patients using coaching techniques to better manage their own health conditions and wellbeing.

EPC teams in Wyre and Fylde are large integrated teams covering either Wyre locality based at Fleetwood or Fylde locality based at Lytham.

The successful post holders will work with others in the team which includes care coordinators, community matrons, district nurses , therapists and health and wellbeing workers. The aim of EPC is to support patients within their own homes in order to make lifestyle and behaviour changes to improve and self-manage their health and wellbeing.
