Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester. The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients. We employ more than 6,500 staff, have approximately 900 beds and have a planned turnover in excess of £410m for 2017/2018.

As one of the United Kingdom’s largest coastal resort, Blackpool has plenty to offer its residents – it’s not just a good place to work; it’s a great place to live. In addition to its bustling centre and historic seafront, the town is world famous for its many attractions, including the famous Blackpool Tower. Blackpool also offers a number of scenic cycling routes round our local parks and across the surrounding countryside, as well as boasting panoramic views of the coast on its picturesque Promenade. Further afield, Blackpool also benefits from less than three-hour connections via rail to Edinburgh in the north and London in the south.

Our strategic vision for 2020 is:

As a high performing Trust, operating as part of an integrated care system, we will provide high quality, safe and effective care. This will be achieved in a financially sustainable way, through our skilled and motivated workforce”.

The REACT service provides rapid assessment of patients identified as having urgent and unscheduled care needs, who could be safely managed in the community. This role is the key component in providing a single point of access for patients into a range of primary, secondary and social care services.

The service supports the Trust’s Unscheduled Care Agenda, providing an alternative pathway to manage patients in their own homes in order to both reduce unnecessary hospital admissions and or facilitate early safe discharge.

An experienced Clinical Specialist [Physio / OT / Nurse] with advanced assessment skills you will work predominantly within the Urgent Care Centre at the Royal Lancaster Infirmary alongside Urgent Care GPs identifying those who could be cared for at home and referring them into appropriate Community Health and Social Care Services – You will also act as assessor for the Rapid Response service receiving referrals directly from GPs or other health and social care professionals working in the community which will involve assessing patients in their own homes, planning and initiating health and social care packages through the Rapid Response Team

This represents an exciting opportunity to be part of a multi-professional service working in collaboration with both Primary and Secondary Care Providers.
Independently managing your own caseload, you will be responsible for interpreting and analysing clinical and non-clinical facts to form an accurate diagnosis, discuss treatment options and agree a management plan with patients. You will also work with a variety of health & social care stakeholders to ensure timely assessment and delivery of a co-ordinated multi-disciplinary / multi-agency service.

The service will provide patients with an alternative care option interfacing with primary and secondary care. Therefore it requires a proactive individual with excellent clinical knowledge, leadership and communication skills.

Informal discussion is very much encouraged.
