Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester. The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients. We have an exciting opportunity for you to join The Community Therapy and Rehabilitation service within North Lancashire.

This service provides adult general community therapy and rehabilitation through physiotherapy and occupational therapy assessment, including falls prevention and risk management. Patients are seen in their own homes and also in residential care and residential intermediate care settings.

Community therapy services are currently aligning to the Integrated Care Community structure, supporting the health needs of the local population, registered to a GP practice, thus enhancing multi-disciplinary team working.

Working in partnership with colleagues from Lancashire County Council, we provide the physiotherapy service to 22 intermediate care beds at Dolphinlee House in Lancaster incorporating 10 dementia rehabilitation beds and 12 rehabilitation beds.   The unit will accept patients who step up from the community and step down from the hospital. The staff will be a multidisciplinary team and work to support patient flow across the health economy.

The community therapy team provide input to the unit to progress clients through their rehabilitation journey and support the transfer of care into the community setting, when clients have ongoing therapy needs.

This post will offer the successful candidate the opportunity to work within a proactive multidisciplinary team with partners from health and social care.

There will be opportunity to rotate within the different clinical areas to gain a broad experience and knowledge base within the different settings.
