Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester. The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients. Palliative and End of Life Care Digital Champion– 8a 0.4 wte (12 months fixed term secondment, potential for extension)

We work together to join up health and care services, listen to the priorities of our communities, local people and patients and tackle some of the biggest challenges we are all facing.

We are clear about what we want to achieve in terms of electronic sharing of palliative and end of life care records across all of the organisations within the partnership: our aim is that all patients who are in the last 12 months of life will have the opportunity for their health care records to be shared across all health and care organisations in Lancashire and South Cumbria. This will mean that clinicians can better consider patient preferences and make more informed decisions.

We are seeking a dynamic clinician to clinically lead this piece of work. With some experience of digital policy and significant experience and strategic understanding of end of life care, you will enable those steps needed to progress to a consistent sharing of clinical and care records for people at end of life to ensure seamless and dignified care.
